Job summary

Empath Health is seeking a Dementia Services Care Navigator to provide psychosocial assessments for people living with dementia and their caregivers within the Nancy Renyhart Dementia Education Program (NRDEP).

As part of a compassionate, mission-driven team across the Florida network, you’ll conduct GUIDE screenings, coordinate referrals, and support families through education and outreach, delivering Full Life Care with empathy and dignity.

Qualifications

• Master’s Degree in Social Work and at least one year of caseworker experience.
• Experience in counseling, crisis management, and grief support preferred.
• Public speaking and group facilitation skills required.
• Ability to work independently and adjust hours to meet program participants and family caregivers.

Responsibilities

• Provides outreach to professionals, community-based organizations and the general community about the NRDEP and the program’s dementia services.
• Utilizes community resources to assist person with dementia and caregiver to resume life in the community or to manage daily life with dementia.
• Develops referral relationships with physicians, dementia diagnostic clinics and other healthcare providers.
• Facilitates Virtual Dementia Tour trainings as needed.
• Facilitates dementia caregiver support groups and ensures positive outcomes.
• Maintains knowledge of Medicare/Medicaid programs and applies that knowledge to qualifications for skilled nursing facilities.
