Job summary

Baystate Health's Care Team Navigator program seeks empathetic communicators to support people living with dementia and their caregivers through video or phone sessions. CTNs receive training and work with a skilled clinical team to coordinate education, safety, and care planning, connecting families with community resources and ensuring medication safety.

No college degree required, Spanish language skills desired; strong communication and social skills are essential for empowering families

Responsibilities

• Provide dementia-specific education and support to the patient and family and help families learn what to expect in the future.
• Screen dyads for unmet care needs (including clinical and medication issues), behavioral issues, safety risks, and psychosocial well-being.
• Working in concert with the MACC clinical team, the CTN guides each dyad through the process of advanced care planning.
• Connect dyads to local community services, including respite care.
• Collect information about medications and medication adherence, present this information to the care team, assist in medication reconciliation, and help implement strategies to improve medication adherence.
• Work with both the care team and the dyad to assess medication side effects and changes in function and behavior.
