Job summary

Baystate Health's Memory Assessment and Care Center seeks a Care Team Navigator to serve as the primary contact for people living with dementia and their caregivers, coordinating with the MACC care team primarily through video or telephone visits. The CTN role does not require a college degree or health care background; strong communication and social skills are essential, with fluency in Spanish preferred.

Training in ethics, aging, dementia care, and advance care planning will be provided.

Qualifications

• Excellent communication and social skills are required.
• Bilingual English/Spanish is preferred but not required.

Responsibilities

• Provide dementia-specific education and support to patients and families.
• Screen dyads for unmet care needs, behavioral issues, safety risks, and psychosocial well-being.
• Guide each dyad through advanced care planning in collaboration with the MACC clinical team.
