Connections in Ohio is seeking a Data Entry Specialist to join our team in San Jose, CA. You will manage inbound documentation, perform data entry, and route information supporting patient scheduling, billing, and clinical workflows in a fast-paced environment.

Responsibilities include processing faxed documents, uploading to patient charts, validating demographics and insurance data, and supporting related administrative projects.

Qualifications

• High school diploma or GED.
• 1 year customer service and/or data entry experience.
• Proficient with MS Office and typing.
• Ability to handle a high volume of calls.
• Preferred: medical terminology background.
• 2+ years of customer service and data entry experience.

Responsibilities

• Document intake & processing of inbound faxed records.
• Upload documents to patient charts and route to departments.
• Locate and validate patient records, resolve duplicates.
• Enter demographic, insurance, and billing details accurately.
• Process orders/referrals with correct modality, codes, urgency.
• Verify provider details and credentialing compliance.
• Follow up with physicians/patients as needed.
• Support cross‑functional projects and training.
