Minn esota Department of Human Services is seeking a Continuity of Care Manager in St. Paul. The role leads the design, development, and implementation of the new Continuity of Care legislation, with emphasis on strategic vision and cross-agency collaboration.

The position requires leadership experience, 4 years in professional roles, and deep knowledge of Medicaid and related services. Telework eligibility is available, with a Monday–Friday schedule and up to 15% travel.

Qualifications

• Leadership experience including supervisory or team lead roles.
• 4 years of professional work experience required.
• Extensive knowledge of Medicaid programs and related services.
• Knowledge of state, county, Tribal and managed care delivery systems.

Responsibilities

• Design, develop, and operationalize Continuity of Care legislation.
• Provide strategic vision and management for comprehensive program requirements.
• Coordinate internal and external collaboration to ensure continuity of care.
• Lead programs addressing transitions, housing, and community supports.
