Job summary

Medica is seeking Care Coordinators—RN or Social Work professionals—to support members in Minnesota with coordinated, home-based care. You’ll develop individualized care plans, educate on benefits, and partner with physicians, providers, and county entities to manage care delivery.

This remote role requires you to reside in MN and allows you to choose a 4- or 5-day work week with a schedule free of evenings, weekends, and holidays. Mileage reimbursement is provided for community visits.

Qualifications

• RN Diploma or Degree in Nursing (Associates or Bachelors) + 2+ years in community-based services
• Bachelor’s in Nursing with a Public Health Certification + 1 year in community-based services
• 3-5+ years of experience beyond degree
• Bachelor’s degree in Social Work
• 3-5+ years of experience beyond degree, including 1-2+ years in community-based services
• Active, unrestricted RN License or SW License in MN

Responsibilities

• Manage a specified caseload and coordinate the delivery of approved medical and social services
• Educate members on benefits including Medicare and Medicaid
• Partner with physicians, providers and county financial entities to discuss care plans and provide updates
• Performs other duties as assigned
