Job summary

Molina Healthcare is seeking a Care Manager to support NM Medicaid members by coordinating care and guiding them through health services. The role blends phone outreach with in-person meetings and requires careful documentation of interactions.

Travel within the local service area is reimbursed. Responsibilities include assessments, care planning, and continuous monitoring to ensure positive outcomes while collaborating with a multidisciplinary team.

Qualifications

• At least 2 years experience in health care, preferably in care management.
• Valid driver's license and reliable transportation for travel.
• Demonstrated knowledge of community resources.
• Ability to operate proactively and demonstrate detail-oriented work.
• Excellent time-management and prioritization skills with ability to manage multiple projects.

Responsibilities

• Completes assessments of members per regulated timelines and determines eligibility for care coordination.
• Develops and implements care plans in collaboration with member and care team.
• Conducts telephonic, face-to-face or home visits as required.
• Performs ongoing monitoring of care plan and documents interventions and outcomes.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing to educate, support and motivate change.
• Assesses barriers to care and assists members to address concerns.
• Collaborates with licensed care managers as needed.
• Travel 25-40% local travel may be required.
