Job summary

Hennepin Healthcare is seeking a Connection Advisor – Outreach to support a fast-paced, high-volume inbound call center. The role entails scheduling appointments, verifying demographics, and ensuring accurate insurance information while delivering exceptional patient service.

This position supports centralized referrals and discharge appointments, with a focus on coordinating care levels and maintaining high-quality interactions in a dynamic environment.

Qualifications

• High School Diploma.
• Two years data lookup/data entry experience.
• Two years in customer service involving complex analytical problem-solving.
• One year of remote work experience.

Responsibilities

• Answers assigned calls; prioritizes, screens, and/or redirects calls as needed.
• Schedules, cancels, and reschedules appointments for patients/callers following standard work and policies.
• Handles complex scheduling across multiple appointments or different providers and modalities.
• Obtains and accurately captures demographic and health insurance information.
• Completes patient registrations in a timely, professional manner.
• Assists with shadowing and mentoring newly onboarded staff.
• Recommends and supports change and process improvements while upholding workflows.
• Uses EPIC work queues and Outlook for timely status updates.
