Tuesday Health in Texas is seeking a Registered Nurse (Texas) - Bilingual, Spanish to lead care navigation for patients with serious illness. You will administer assessments, develop care plans, and coordinate rounds using our electronic systems to deliver coordinated, home-based care.

The ideal candidate holds an active Texas RN license, is fluent in Spanish, and has clinical or multidisciplinary care experience.

Qualifications

• Active and unrestricted RN license in Texas.
• Bilingual in Spanish (spoken and written).
• Experience in clinical/medical settings preferred.

Responsibilities

• Administer assessments and screening tools within the clinical care model.
• Interpret assessment results to prioritize patient needs.
• Lead care plan development within a multidisciplinary team.
• Coordinate rounds to optimize member experience and outcomes.
• Learn and use Tuesday Health electronic systems and tools.
