Job summary

Qualifications

• Requires a High School diploma or GED.
• Requires 1–2 years of related experience.
• Experience with social determinants of health (SDOH) barriers is preferred.

Responsibilities

• Supports community connection activities and connects members to community resources.
• Provides telephonic support to foster care members and coordinates care resources.
• Documents and maintains community resources in accordance with health plan requirements.
• Conducts non-clinical health assessments and refers members to appropriate services.
• Assists clinical team of nurses and social workers and supports outreach and education.
