Job summary

Centene Corporation in Michigan seeks a Care Manager to assess member needs, coordinate care plans, and connect families with community resources. This role offers telephonic support to foster care children, with a Monday–Friday 8:00 am–4:30 pm schedule and the flexibility to work remotely from anywhere within Michigan.

You will evaluate barriers to care, develop personalized care/service plans, and collaborate with providers to ensure timely access to services.

Qualifications

• Requires a Bachelor's degree and 2–4 years of related experience.
• Graduate from an Accredited School of Nursing if holding clinical licensure.
• Current state's clinical license preferred.

Responsibilities

• Evaluates the needs of the member, barriers to care, the resources available, and recommends and facilitates the plan for the best outcome.
• Develops or contributes to the development of a personalized care plan/service ongoing care plans/service plans and identifies providers, specialists, and community resources needed for care.
• Provides psychosocial and resource support to members/caregivers and care managers to access local resources or services.
• Coordinates between the member/family and the care provider team to ensure care or services are accessible in a timely manner.
• Monitors progress towards care plan goals and collaborates with providers for revisions as appropriate.
• Collects, documents, and maintains all member information to ensure compliance with regulations.
• May perform on-site visits to assess member needs and collaborate with providers or resources.
• May provide education to care managers and members/caregivers on procedures, instructions, and benefits.
• Other duties as assigned by people leader; complies with policies.
