Job summary

Vaya Health seeks a TCL Community Liaison to educate members, providers, and community partners about the Transition to Community Living (TCL) program and RSVP processes. You will educate on eligibility, barriers, and access to services, working closely with the TCL team and local hospitals.

You will travel within NC, maintain residency, and operate mostly in the community; assist with referrals, documentation, and quarterly collaborations.

Qualifications

• Bachelor's degree in a relevant field as required.
• Four years of related human services program experience.
• Master's degree with two years of supervised clinical experience preferred.
• Residency in North Carolina or within 40 miles of NC border is required.

Responsibilities

• Educate members/recipients and stakeholders about TCL and RSVP processes.
• Collaborate with TCL team and community partners to identify needs and barriers.
• Set up and facilitate meetings with community stakeholders regarding TCL and RSVP.
• Document interventions clearly in the consumer's health record and reports.
• Coordinate across LME/MCO systems to ensure appropriate services and care.
