Job summary

This role requires residency in North Carolina or within 40 miles of the NC border and involves travel in the catchment area.

The position emphasizes collaboration with the TCL team, documentation accuracy, and contributing to reporting for the DOJ settlement. Strong communication and problem-solving skills are essential.

Qualifications

• Bachelor’s degree in public service administration, psychology, sociology, or social work preferred.
• Master’s degree in related human services field with supervised clinical experience or equivalent.
• Experience in case management, assessment and referral is valued.

Responsibilities

• Educate individuals, recipients and providers about TCL eligibility and RSVP process.
• Coordinate with referral entities to assess needs and educate on services and levels of care.
• Document interactions clearly in the consumer's health record and support reporting requirements.
