Job summary

Munson Healthcare in Michigan is seeking a Community Health Worker for the East Region, with a requirement to live near Indian River, Grayling, Gaylord, or Mio. The role is full-time, day shift, 40 hours per week, Monday through Friday, 8 AM to 5 PM.

The CHW will collaborate with patients, families, primary care teams, and community organizations to improve access to care and address social drivers of health, while guiding care plans and connecting patients to resources.

Qualifications

• High School Diploma or GED is required.
• _completion of a Michigan-recognized CHW training program is required or willingness to complete.
• MiCHWA CHW certification or eligibility is highly preferred.
• Experience in health care, public health, public health education, care coordination, or related fields is desirable.
• Bilingual skills are preferred where relevant.

Responsibilities

• The CHW serves as a frontline public health worker partnering with patients, families, primary care teams, and community organizations to improve access to care and connect patients with resources.
• Provide health system navigation, outreach, education, care plan reinforcement, and culturally responsive coaching; does not provide clinical assessments or licensure-requiring services.

Education
