Job summary

Qualifications

• High School Diploma or GED required.
• 3+ years of experience in Community Health setting.
• 2+ years of experience in local community with knowledge of culture and resources.
• 1+ years of case management experience.
• Intermediate knowledge of Medicare and Medicaid benefits.
• Proficiency with Microsoft Office applications.
• Access to reliable transportation and valid US driver’s license for travel.
• Ability to travel up to 10% of the time.
• Home workspace with secure PHI and reliable high-speed internet.

Responsibilities

• Partner with the clinical care management team to manage complex members in the community.
• Locate and engage members in the community.
• Conduct screenings to assess medical, behavioral, functional, cognitive, and psychosocial needs.
• Research and refer to community resources to address care or socioeconomic needs; follow up on services.
• Support HEDIS/STAR gap closure with scheduling reminders and verification of appointments.
• Provide education and coaching to promote self-management as per care plan.
• Collaborate with internal care team, community partners and providers.
• Consult with clinical team based on defined triggers.
• Assist access to Medicaid benefits to support care needs.
• Help develop personal wellness and health crisis plans.
