Job summary

CVS Health Corporation is seeking a Community Health Worker (CHW) in Louisiana to serve as a bridge between members and the healthcare system. This full-time remote role requires 60–75% travel within-state, supporting Aetna members with chronic conditions and assisting with care plans, education, and accessing community resources.

Qualifications

• High school diploma or GED. Community Health Worker/Promotora certification within 12 months of start date.
• CHW certification program recognized by the State of Louisiana within 12 months of start date.
• Ability to travel 60-75% of the time with reliable transportation.
• 3-5 years' relevant work experience; 1-2 years clinical support experience; 1 year outreach experience.
• Familiarity with LA community resources; experience with clients with clinical needs.
• Proficient in MS Office Suite and basic computer skills; willingness to work outside core hours.

Responsibilities

• Conduct home visits with members to reinforce care plans and address cultural and socio-economic needs.
• Educate providers on social determinants of health impacting member care and treatment plans.
• Build positive relationships with members, providers, case managers and staff from diverse backgrounds.
• Assist with care plan implementation, health and social management plans, and linkages to community services.
• Identify barriers and assist with applications for eligible programs and transportation services.
• Support medication adherence and disease self-management; accompany members to appointments as needed.
• Maintain member records in electronic health records and document progress notes and reports.
• Travel extensively to client homes and community locations as required.
