Job summary

Optima Medical Management Group seeks a Community Health Worker (CHW) to serve as a trusted liaison between community members, healthcare providers, and social services. Working mostly remotely as part of the Care Team, the CHW conducts outreach, provides health education, and connects members to medical, behavioral health, and community resources.

The role requires a high school diploma or GED, CHW certification, and a valid California driver's license.

Qualifications

• High school diploma or GED required.
• CHW certification required.
• Valid California drivers license and vehicle insurance required.
• Experience in case management, member care or customer service preferred.
• Excellent communication and cultural sensitivity.

Responsibilities

• Conduct in-person visits and provide health education.
• Assist access to healthcare services and community resources.
• Assess social determinants of health and identify barriers.
• Support care coordination with providers and social workers.
• Travel to appointments when needed and accompany members.
• Document interactions in designated systems.
• Maintain confidentiality and comply with regulations.
• Participate in community outreach events and health fairs.
• Report to office as needed for meetings.
