Job summary

• Maintain a valid driver’s license with current auto liability insurance.

Responsibilities

• Supervise non-clinical CHWs and ensure outreach, care coordination, and health education efforts meet program goals.
• Monitor performance metrics and conduct regular coaching, field observations, and evaluations.
• Develop and implement CHW workflows, processes, and programs in collaboration with health equity leadership.
• Travel within the region/state to support CHW activities and member engagement.
• Attend departmental and corporate meetings and facilitate training for CHWs.
