Job summary

The Health Partnership seeks an Integrated Community Care Coordinator to help Routt, Moffat, and Rio Blanco residents access health and social services. You will build relationships with clients, coordinate care across providers, and ensure timely documentation in the EHR.

Fluency in English and Spanish is preferred. This role requires strong organizational skills, empathy, and the ability to work across rural communities with a flexible schedule.

Qualifications

• High school diploma required; bachelor’s degree preferred.
• Fluency in English and Spanish preferred.
• Experience in healthcare, human services, or care coordination preferred.
• Ability to communicate effectively with diverse populations.
• Strong organizational and documentation skills.

Responsibilities

• Conduct outreach and assess Medicaid/Medicare residents.
• Coordinate referrals to healthcare, social services, and community resources.
• Develop personalized, goal-oriented care plans.
• Document interactions in EHR promptly.
• Travel within Routt, Moffat, and Rio Blanco counties as needed.
