Job summary

Health Partnership in Northwest Colorado is seeking an Integrated Community Care Coordinator to help Medicaid/Medicare clients access care and resources. The role emphasizes building relationships with healthcare providers and community partners to reduce barriers and support person-centered care.

You will develop goal-oriented care plans, assess client needs, and document progress while traveling regionally.

Qualifications

• Fluency in English and Spanish preferred.
• Experience in healthcare, human services, community health, care coordination, or social services preferred.
• Ability to communicate effectively with clients, community members, healthcare providers, partner organizations, funders, and government agencies.
• Proficiency with Microsoft Outlook, Word, Excel, Teams, and PowerPoint.

Responsibilities

• Conduct outreach and assess Routt, Moffat, and Rio Blanco County residents enrolled in Medicaid and Medicare.
• Coordinate referrals to healthcare, behavioral health, social services, and community resources.
• Educate clients and develop comprehensive, goal-oriented care plans.
• Document interactions and care plans in the EHR system in a timely manner.
