Job summary

The Health Partnership is seeking a Regional Health Connector to build and strengthen partnerships across Northwest Colorado, coordinating with primary care, behavioral health, public health, and community organizations.

The role emphasizes equity, collaboration, and proactive outreach. This part-time, hourly position requires travel across Routt, Moffat, Rio Blanco, Grand, and Jackson counties with regular in-person engagement.

Qualifications

• Bachelor’s degree in a health-related field, public health, social work, health administration, or related field.
• Experience with community-based health programs and providers.
• Demonstrated ability to convene stakeholders, facilitate meetings, and build consensus.
• Excellent interpersonal, written, and verbal communication skills.
• Ability to travel regionally and statewide for required meetings.

Responsibilities

• Build and maintain relationships with community-based organizations, local public health and human services providers, primary care and behavioral health partners.
• Convene partners and participate in meetings, coalitions, and regional events that align with the RHC role.
• Facilitate collaboration and coordination among health care providers and community organizations to strengthen clinic-community linkages.
• Identify regional needs, assets, and barriers, gathering partner feedback to inform planning.
• Develop and update implementation plans to support community linkages and statewide priorities.
