Qualifikationen

• Bachelor's degree in social work, psychology, counseling preferred.
• Experience in community engagement, referral generation, care management preferred.
• Bilingual Spanish preferred depending on location.
• Valid driver's license and auto insurance required.
• Willingness to travel regionally to homes/communities.

Aufgaben

• Provide comprehensive care management services to individuals, ensuring access to necessary support for step-down care from hospitals and other facilities.
• Develop individualized service plans with clients and clinicians.
• Coordinate with external service providers and community resources for seamless transitions.
• Lead community outreach and referral generation within identified Counties.
• Ensure outreach activities comply with laws and ethical standards.
