Job summary

Harbor is seeking a Care Coordinator to perform comprehensive, multi-system care coordination for youth with complex needs and their families. This full-time remote role supports communities, with evening and weekend availability to meet families where they are.

You will build care plans, link families to resources, and collaborate with providers to ensure services are delivered per plan, while maintaining documentation and communication across systems.

Qualifications

• Minimum 3 years of experience in providing community-based services to youth and families, or 2 years with an Associate or Bachelor degree, or 1 year with a Master degree.
• Experience in family systems, community resources, case management, counseling or therapy, child protection, or child development.
• Must be able to communicate clearly with diverse audiences and represent Harbor in the medical/healthcare community.
• Must have valid driver's license and acceptable driving record; insurable by Harbor.
• CPR/First Aid and NCI/CPI certifications required within 90 days of employment.

Responsibilities

• Engage youth with significant behavioral health needs and their families for assigned caseload.
• Link youth and family to community resources and supports.
• Conduct comprehensive CANS assessment and develop an individualized care plan.
• Monitor and adjust the plan as needed to address challenges and barriers.
• Facilitate Child and Family Team meetings ensuring youth and family have a voice.
• Coordinate access to clinical and psychiatric consultation for assigned youth.
• Develop safety-crisis plans and ensure documentation complies with CME rules.
• Work flexible hours including evenings and weekends; provide services in home and community settings.
