Job summary

Harbor is seeking a Care Coordinator to perform comprehensive, multi-system care coordination for youth with complex needs and their families. This full-time role (40 hours/week) is remote and community-based, with preference for candidates living in Ottawa or Sandusky, OH.

You will conduct CANS assessments, develop individualized care plans, and coordinate across child-serving systems to keep youths integrated in the community.

Qualifications

• At least 3 years of experience (or 2 years with an Associate or Bachelor degree; or 1 year with a Master degree) in providing community-based services and supports to children, youth and families.
• Experience in family systems, community resources, case management, child development or related public sector human services.
• Must be culturally competent to manage complex cases and work with diverse families.

Responsibilities

• Engage children/youth with behavioral health needs and their families for assigned caseload (1:10 for ICC and 1:25 for MCC).
• Link youth and families to community resources and supports.
• Conduct CANS assessments and develop individualized, strength-based Care Plans.
• Monitor plans and address challenges; update plans as needed.
• Facilitate Child and Family Team meetings and ensure family voice and choices.
• Coordinate access to clinical and psychiatric consultation for youth.
• Develop safety/crisis plans with youth and family and review.
• Complete engagement, contacts, and documentation per CME rules.
• Work flexible hours including evenings/weekends.
• Provide services in home and community settings.
