Job summary

Harbor seeks a CME Hospital Liaison to coordinate care for youth in the CME program through hospital partnerships and care coordination. The role involves admission/discharge collaboration to ensure timely access to services and continuity of care.

It is a full-time position that works remote and in the community, with responsibilities including outreach, safe transitions, and ongoing care coordination across multiple child-service systems.

Qualifications

• At least 2 years of experience in behavioral health serving youth and families.
• Experience in family systems, community resources, case management, or therapy.
• Cultural competence for complex cases.
• Experience with youth in SED, trauma, and multi-system involvement.

Responsibilities

• Visit hospitals regularly to strengthen relationships and engagement.
• Coordinate services for clients discharging from hospitals.
• Schedule follow-up to ensure continuity of care up to 30 days post discharge.
• Monitor relevant reports and communicate with discharge planners.
• Ensure safe transitions to community-based care before discharge.
• Address barriers to care including transportation and housing.
• Maintain regulatory documentation compliance.
