Job summary

Jaan Health is seeking Certified Medical Assistants (CMA) Clinical Care Coordinators to join our care team and report to the Clinical Operations Manager. They will support patient-centered, team-based care and assist PCPs in managing panels using population management tools.

The role focuses on high-volume outreach via calls, texts, and platform messaging, ensuring accurate documentation and seamless workflow within the care team.

Qualifications

• Certified Medical Assistant required.
• Working PST Time Zone.
• Minimum of 3 years experience in care management at a primary care setting or similar; population health experience preferred.
• Excellent communication and technology skills.
• Prior use of electronic health records and other health care information systems.
• Significant experience in a primary care setting (chronic care preferred).
• Proven problem-solver with ability to manage high-volume outreach and multi-tasking.

Responsibilities

• Conduct high-volume patient outreach via phone, text, and digital platforms.
• Document patient interactions and care management activities in EHRs (EPIC, Cerner, Allscripts, eCW, etc).
• Support clinical staff with intake coordination, appointment scheduling, and medication refills; escalate clinical issues as appropriate.
• Assist with patient onboarding and enrollment, and partner with client operations teams to ensure seamless service delivery.
• Deliver training sessions on care protocols, systems, and operations to care management teams.
• Adapt to evolving workflows, technology, and program demands.
• Contribute to a positive patient and family experience through professional interactions and appropriate referrals.
