Job summary

CareStar, Inc. is seeking a Clinical Supervisor for the Specialized Recovery Services Program in Ohio. You will assess needs, develop care plans, coordinate services, and supervise care team members.

Remote with field visits, contributing to high-quality care coordination for high-need populations. Ideal candidates are licensed RNs or licensed social workers with CCM certification and at least five years of clinical experience in home- and community-based services, including supervisory

Qualifications

• Must be licensed RN or licensed social worker (LSW/LISW) with at least five (5) paid years of clinical experience in home- and community-based services and a minimum of one (1) year of supervisory experience.
• CCM certification required or to be obtained within one year of placement.
• Experience in completing assessments, service plan development and monitoring.
• Effective organizational and communication skills.
• Experience providing care management/coordination preferred.

Responsibilities

• Integrity and self-discipline with the ability to work independently within guidelines.
• Knowledge of Medicare, Medicaid, insurance and waiver programs and community resources.
• Good problem-solving skills.
• Understands and complies with CareStar Policies and Procedures.
• Maintains confidentiality as related to patient information.
• Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
