Job summary

CareStar, Inc. is seeking a Clinical Supervisor to lead the Specialized Recovery Services program in Ohio.

This role focuses on assessing patient needs, developing tailored care plans, and connecting individuals with essential community services and supports. Ideal candidates will hold an active RN or social work license and CCM certification, with at least five years of clinical home-and-community care experience and one year of supervisory responsibility.

Qualifications

• Must be licensed as RN, social worker, or LISW with at least 5 years of clinical home-and-community based experience and at least 1 year of supervisory experience.
• CCM certification required or obtained within one year of starting.

Responsibilities

• Assess client needs and develop personalized care plans.
• Coordinate services and supports to meet client goals.
• Maintain client confidentiality in all information exchanges.
• Adhere to CareStar policies and procedures.
• Conduct assessments and monitor service plans throughout care.
• Support patients with care management/coordination as needed.
