The ideal candidate has licensed clinical credentials, 5+ years of home- and community-based experience, and supervisory capability. This full-time role offers remote flexibility with field visits and supportive benefits.

Qualifications

• Licensed RN or LSW/LISW with 5+ years in home- and community-based services.
• CCM certification required within 1 year of hire.
• Experience in completing assessments, service plans, and monitoring.
• Effective organizational and communication skills.
• Experience in care management/coordination preferred.

Responsibilities

• Adheres to CareStar policies and procedures.
• Directly supervises staff including hiring, firing, and evaluations.
• Provides direct clinical services to consumers as needed.
• Educates, counsels, evaluates, and manages performance of the clinical team.
• Maintains knowledge of Medicare, Medicaid, insurance and waivers.
• Performs other projects and duties as assigned.
