Job summary

Optum is seeking a Clinical Practice Performance Coordinator to liaison between patients, emergency departments, and care teams, ensuring compliant documentation and real-time coordination across the patient journey. This role emphasizes CMS/NCQA compliance, data entry, and outstanding customer service.

You will work a 40-hour week with multiple shifts and may telecommute from anywhere in the U.S., supporting program needs, handoffs to APCs, and process improvements while maintaining PHI

Qualifications

• High School Diploma or GED required.
• Medical assistant certification required.
• 2+ years administrative support experience.
• Knowledge of medical terminology.
• Ability to work listed shifts.
• Must secure PHI and work from a designated home workspace.

Responsibilities

• Document all member interactions in a timely and concise manner per Model of Care, NCQA and CMS requirements.
• Respond to incoming ADT notifications per training.
• Make inbound/outbound calls for program requirements and ED outreach.
• Enter data into software applications accurately.
• Provide clerical support to clinical staff and managers.
• Schedule appointments to support RN care coordination.
• Participate in required meetings and adhoc committees.
• Identify opportunities for process improvement in member care.
