Job summary

Post Acute Analytics Inc. seeks a Clinical Program Manager to operationalize clinical programs across health plan and healthcare partners. You will translate strategy into workflows, coordinate implementation, and monitor performance to ensure timely action.

In a remote-first US-based team, you will partner with clinical leadership, Product, Operations, and client teams to drive adoption, measure outcomes, and improve care delivery across post-acute care settings.

Qualifications

• Bachelor-level degree in a relevant health field and active clinical license preferred.
• Minimum five years in healthcare with clinical program implementation or operations.
• Experience with payer/provider workflows and regulated environments.
• Ability to manage multiple workstreams with stakeholders and deadlines.

Responsibilities

• Translate clinical strategy into executable workflows and actionable plans.
• Coordinate readiness, education, launch activities, and stabilization.
• Maintain program plans, risks, decisions, and performance reporting.
• Provide day-to-day clinical program management for payer and provider programs.
• Collaborate with Product and technical teams on workflow needs and usability.
