Qualifikationen

• 4+ years of experience as a clinical MTM pharmacist (management experience preferred).
• Familiarity with Medicare Part D Star measures, specifically SUPD, SPC, and CMR.
• Clinical experience in medication review.
• Strong medical/pharmaceutical vocabulary and documentation accuracy.
• Strong verbal and written communication skills.
• Must be tech savvy; Salesforce experience preferred.

Aufgaben

• Conduct outbound phone calls (high call volume up to ~100 calls/day).
• Complete Comprehensive Medication Reviews (CMR).
• Deliver Medication Therapy Management (MTM) services.
• Provide in-depth consultations on statin medication use per AHA guidelines.
• Accurately and efficiently transcribe prescription, diagnosis, and recommendation data into a third-party, web-based application.
• Attend training sessions to enhance MTM/CMR-related skills.
