Job summary

Guidehealth is seeking a Clinical Pharmacist with a PharmD to lead HEDIS medication adherence initiatives, analyze adherence data, and collaborate with care teams to improve outcomes. This role uses EHRs and data tools to monitor performance and drive quality improvement.

You will conduct MTM sessions, address barriers to adherence including cost and dosing, participate in cross-functional quality programs, and support training for staff while maintaining licensure across multiple states.

Qualifications

• Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.
• Active pharmacist license, willing to obtain licenses in TX, TN, AL and PA (company paid).
• Minimum of 2 years of experience in HEDIS-related medication adherence work, quality improvement, or population health.
• Strong knowledge of HEDIS measures, particularly those related to medication adherence.
• Proficiency in using EHR systems and data analysis tools.
• Excellent communication, problem-solving, and organizational skills.
• Ability to work independently as well as in a team-oriented environment.
• Experience with motivational interviewing or other patient engagement techniques is a plus.

Responsibilities

• Lead efforts to improve HEDIS medication adherence and other pharmacy quality measures.
• Analyze medication adherence data, identify gaps in care, and implement targeted interventions.
• Collaborate with healthcare providers, care managers, and other stakeholders to promote adherence.
• Conduct MTM sessions to address barriers to adherence and educate patients.
• Use EHR and pharmacy data to identify at-risk patients and develop action plans.
• Provide regular updates to leadership on progress towards HEDIS goals.
