Job summary

Carefeed is seeking a Clinical Implementation Specialist – Referral Management to bridge clinical operations and technology implementation for skilled nursing and post-acute care clients. You will guide deployments, train teams, and establish repeatable workflows across central intake, admissions, MDS, and business offices.

You will partner with Product and Support to optimize workflows, drive adoption, and ensure successful go-live while protecting patient and resident privacy.

Qualifications

• Five+ years in skilled nursing/post-acute operations with central intake & authorization.
• Experience coordinating referrals and reviews with payers.
• Knowledge of clinical docs, MDS, PDPM, and reimbursement.
• Collaborating with discharge planners, payer case managers, admissions, nursing, and office teams.
• Strong communication, facilitation, and organizational skills; ability to train others.
• Comfort learning new technology and handling confidential information.

Responsibilities

• Lead client discovery and workflow planning for central intake and referrals.
• Translate needs into module usage and define careflow between Carefeed and EHRs/portals.
• Deliver role-based training for central intake, admissions, nursing, MDS, therapy, and business teams.
• Manage go-live, milestones, actions, and handoffs to support teams.
• Serve as SME on skilled nursing referrals, documentation, and managed care workflows.
• Create repeatable resources: training materials, guides, checklists, SOPs.
• Collaborate with Product, Client Experience, and Support to improve workflows.
