Qualifikationen

• Provide guidance and support to clinical members, helping them understand and utilize their healthcare benefits and resources.
• Conduct health assessments, review medical history, and identify at-risk individuals for targeted coaching.
• Track member progress, document sessions, and report outcomes in the EMR.
• Collaborate with the PeopleOne Health Clinical Care Team to create individualized care plans and promote adherence to evidence-based recommendations.
• Engage in education, outreach, and care navigation to support disease management and health behavior change.
