PeopleOne Health, Inc. is seeking a Clinical Health Coach to deliver evidence-based health coaching and clinical guidance to clinical members. The role blends clinical expertise with personalized coaching, focusing on disease management, navigation of care, and behavior change.

The position is mainly remote with a hybrid component, joining team meetings and trainings as needed. You will work with the Clinical Care Team to develop care plans and support members through coaching plans and goal

Qualificações

• Bachelor's degree in a clinically focused health-related field and at least 2 years of clinical experience.
• Clinical licensure or certification (e.g., RN, RD, mental health professional) preferred.
• Active certification in fitness, wellness, or related clinical areas (e.g., NBC-HWC, CDE, CHES) preferred.

Responsabilidades

• Provide guidance and support to clinical members, helping them understand and utilize their healthcare benefits and resources.
• Conduct health assessments, review medical history, and identify at-risk individuals for targeted coaching.
• Track member progress, document sessions, and report outcomes in the EMR.
• Collaborate with the Clinical Care Team to create individualized care plans and promote adherence to evidence-based recommendations.
• Coach through education, outreach, advocacy, and navigation of the healthcare system.
• Stay updated with clinical operations protocols and disease/condition management.
