CVS Health—ABH-LA is seeking a Clinical Case Manager Behavioral Health to support Louisiana residents via telework, with in-state travel for face-to-face visits. Requires a master’s degree and an independent clinical license (LCSW/LPC/LMFT).

Qualifications

• Louisiana resident with independent clinical license (LCSW/LPC/LMFT).
• 3+ years direct clinical practice post-master’s in youth/pediatrics.
• 2+ years MS Office and multi-system navigation experience.

Responsibilities

• Assess members and determine approach to case resolution based on benefit plans and services.
• Apply clinical judgment to reduce risk factors and address complex indicators in care planning.
• Perform crisis intervention and refer to appropriate clinical providers.
• Provide crisis follow-up to ensure appropriate treatment.
• Coordinate with supervisors/Medical Directors; present cases at case conferences for multidisciplinary input.
• Develop and monitor care plans with member and care team; ensure compliance with guidelines.
