Job summary

Caritas of Austin is seeking an AT HOME Clinical Case Manager in Austin, TX to support a multidisciplinary Housing First program. You will assess, plan, and provide therapeutic services to clients with complex needs in their homes, coordinating with the housing team and community resources to promote stability and well‑being.

The role requires at least a bachelor’s degree, relevant licensure, and 1+ year of experience. A Master’s degree is preferred.

Qualifications

• Bachelor’s degree from an accredited college or university.
• Licensure must be obtained prior to or within 90 days of hire.
• Master’s Degree from an accredited college or university in human services (preferred).
• Minimum one year of professional related experience.
• Past work in housing programs or with homelessness; experience with mental health disorders preferred.

Responsibilities

• Provide case management and advocacy for an assigned group of clients with multidisciplinary team.
• Develop, implement, evaluate, and revise individualized service plans with clients.
• Provide regular face‑to‑face meetings to monitor progress toward self‑sufficiency.
• Assess and monitor client mental health symptoms; adjust treatment plans as needed.
• Coordinate housing and access to community services and supports.
