Job summary

Lucet is hiring for a Clinical Care Manager who will oversee comprehensive care management, coordinate behavioral health services, and engage members to improve outcomes. The role emphasizes strong clinical judgment, meticulous documentation, and collaboration with care teams.

Applicants must hold an unrestricted state license and have 3–5 years of relevant clinical experience in behavioral health settings. Remote work flexibility is role-dependent.

Qualifications

• Licensed to practice in state(s) with unrestricted status.
• 3–5 years direct clinical care in behavioral health or substance use settings.
• Strong clinical judgment and case management skills.
• Excellent verbal and written communication; empathetic approach.
• Ability to document in real time while engaging with members.
• Ability to pass comprehensive background checks and screenings.

Responsibilities

• Conduct holistic assessments and develop individualized care plans across the continuum of care.
• Coordinate behavioral health services, transitions, and aftercare with members, providers, and care teams.
• Engage members and providers to improve engagement and continuity of care via motivational interviewing and education.
• Ensure timely documentation and service authorization per medical criteria and policies.
• Participate in audits, quality initiatives, and ongoing professional development to improve care delivery.
