The Colorado Department of Human Services (CDHS) seeks a Clinical Care Coordinator to provide high-quality behavioral health care coordination for Civil and Forensic clients. The role requires navigating jail and hospital settings, interfacing with courts and care partners, and ensuring timely access to services and benefits across Colorado.

You will work remotely from a home office with travel for in-person meetings, primarily near Colorado Springs, subject to change.

Formación

• Current, valid Colorado licensure in one of the listed behavioral/mental health professions.
• At least one year of licensed experience in direct care behavioral/mental health treatment.

Responsabilidades

• Provide therapeutic interventions within scope of practice and licensure.
• Coordinate care and treatment plans for individuals in jail and hospital settings.
• Educate and consult with other staff; participate in individual patient plans of care when needed.
• Interpret clinical results and determine needed services and placements.
• Document clinical encounters, treatment decisions, and progress in the patient record.
• Coordinate access to services (medication management, mental health care, substance abuse services).
• Maintain contact with Care Coordination Program, jails, courts, and hospitals to ensure continuity of care.

LCSW / LPC / LMFT / LAC licensure in CO
