Job summary

Pacific Health Group is seeking a CHW Program Manager to lead a team of CalAIM Community Health Workers delivering whole-person care to Medi-Cal members in San Diego County, CA. You will ensure high-quality service, maintain billing compliance, and supervise daily operations in a remote work setting.

You will coordinate onboarding, provide training, and collaborate with RNs and local partners to optimize care coordination and documentation across multiple systems.

Qualifications

• Bachelor’s degree in Public Health, Social Work, Healthcare Administration, or related field preferred.
• 3–5 years of experience in community health, Medi-Cal services, or social work, with at least 2 years in supervision.
• Valid California Driver’s License with an operable vehicle, clean MVR, and active auto insurance. CHW certification preferred.

Responsibilities

• Oversee, mentor, and evaluate a team of CalAIM CHWs.
• Ensure CalAIM billing compliance and proper CPT coding.
• Audit Plans of Care and progress updates in EHR systems.
• Coordinate onboarding and continuing education.
• Collaborate with RNs and local community organizations.
