Pacific Health Group is seeking a CHW Program Manager to lead a CalAIM CHWs team delivering whole-person care to Medi-Cal members across Humboldt County, CA with a remote work option. You will ensure high-quality service delivery, billing accuracy, and adherence to documentation standards.
The role requires 3–5 years in community health or social work, plus supervisory experience and knowledge of CalAIM policies. A California driver’s license and CHW certification are preferred.

Employment Type: Full-Time; Exempt
Compensation: $78,000-$82,000 Annually
