Job summary

The Cardiovascular Services of America (CVAUSA) is seeking a Health Coach to support heart failure patients within Novocardia's value-based care model. This remote, US-based role focuses on education, lifestyle coaching, and patient engagement while working with the clinical care team to empower self-management.

The ideal candidate has NBC-HWC certification, a health sciences degree, and telehealth experience, with strong communication skills and bilingual English/Spanish preferred.

Qualifications

• Bachelor’s degree in health sciences or related field.
• NBC-HWC certification required.
• Experience in a healthcare, chronic disease management, or patient engagement setting preferred.
• Cardiology or heart failure experience a plus.
• Telehealth or remote patient engagement experience preferred.
• Bilingual English/Spanish proficiency preferred.

Responsibilities

• Provide education and coaching related to heart failure, lifestyle modification, medication adherence, nutrition, physical activity, and self-management.
• Establish short- and long-term goals with patients using shared decision-making and motivational interviewing techniques.
• Encourage sustained patient engagement, self-advocacy, and behavior change throughout program participation.
• Partner with the clinical care team to support patient care plans and elevate concerns appropriately.
• Provide timely updates and documentation related to patient engagement and progress.
• Participate in interdisciplinary team meetings.
• Respond to patient communications in a timely, professional, and compassionate manner.
• Build and maintain strong, trust-based relationships with patients and caregivers.
• Consistently deliver a high-quality patient experience aligned with program standards.
