Qualifications

Responsibilities

• Conduct inbound and outbound coaching calls to enroll participants.
• Provide behavior change coaching using motivational interviewing.
• Coordinate care delivery with nurses and external vendors.
• Maintain caseload and document progress in software.
• Participate in daily and monthly team meetings.
• Track participant progress and program efficacy.
