• Strong verbal and written communication with members and staff.
• Ability to travel within the assigned territory and holds a valid driver’s license.

Responsibilities

• Provide care coordination to members via phone or field visits.
• Educate members on health programs and address personal health goals.
• Conduct health screenings, assessments, and care planning, including home visits.
• Support clinical operations with provider and member activities.
