Job summary

Texas Children's Hospital is seeking a Social Worker to assess, plan, implement, monitor, and evaluate care needs for Health Plan members. You will provide ongoing case management to high risk members, coordinating services across the continuum of care to optimize outcomes, quality, and cost effectiveness.

The role involves collaboration with families, providers, and an interdisciplinary team to ensure comprehensive, patient-centered care and adherence to Medicaid and managed care policies.

Qualifications

• Master's Degree in Social Work is required.
• Licensure: LMSW, LCSW or LMSW-AP as approved by the TBHEC is required.
• Minimum of 2 years clinical experience.
• Authorization to Release Personal Information must be signed prior to starting.

Responsibilities

• Assess, develop, implement and monitor a comprehensive care plan in collaboration with an interdisciplinary team.
• Identify and refer members in need of case management based on clinical rounds and referrals.
• Assess biophysical, psychosocial, environmental needs and financial status of members.
• Coordinate services across the continuum of care and document plans in the system.
• Advocate for members and families across the continuum of care and ensure continuity of care.
