Qualifications

• Bachelor’s degree in nursing or master’s in social work/behavioral health (preferred).
• WA licenses: RN, Social Worker, or Mental Health licensed professionals (required).
• Minimum 1 year case management, home health, discharge planning or equivalent experience.
• Minimum 3 years clinical experience in acute or outpatient settings (required).
• Case Management Certification (preferred).
• Bilingual abilities (preferred).

Responsibilities

• Provide telephonic case management for members with acute, chronic, and complex needs.
• Advocate for members and coordinate resources to improve outcomes.
• Collaborate with care teams, providers, caregivers, and community resources.
• Assess, plan, implement, document care using the organization’s systems.
• Lead and participate in interdisciplinary care team meetings.
• Track progress and adjust care plans to meet goals.
• Document all case activity per policies and protocols.
• Coordinate transitions of care and liaise with providers' network.
