Job summary

Cambia Health is seeking a Remote Care Management Nurse to support members from home across eligible states, including Washington and Oregon. This role emphasizes case management, clinical assessment, and collaboration with care teams to deliver quality, cost-effective outcomes.

Ideal candidates hold a nursing degree and RN licensure, with at least 3 years of relevant experience, and must be eligible for licensure in multiple states.

Qualifications

• Nursing or health/human services degree (AS/BAS) or equivalent experience.
• Minimum 3 years of direct clinical care or case management related work.
• Active, unrestricted licensure to conduct assessments; RN license required for medical care management.
• Eligible for licensure in Idaho, Oregon, Utah and Washington.

Responsibilities

• Perform case management activities across assessment, planning, implementation, coordination, monitoring and evaluation.
• Apply clinical judgment to ensure medical policy and necessity guidelines are met.
• Collaborate with physicians, internal/external stakeholders to resolve issues and improve quality of care.
• Serve as a resource, responding professionally while maintaining confidentiality.
• Maintain accurate documentation and meet performance/timeline standards.
• Coordinate resources and prioritize assignments to achieve goals.
• Monitor effectiveness of case plans and adjust as needed.
