Job summary

The Columbus Organization, LLC is seeking a dedicated Case Manager to coordinate care in Windham County, VT, with hybrid work and required travel. The role covers assessment, planning, and monitoring in line with state standards and agency policies.

Responsibilities include coordinating planning meetings, supporting individual goals, and ensuring documentation is complete and accessible to a multidisciplinary team.

Qualifications

• A bachelor’s degree and one year of human services experience, or an associate degree with two years, or a high school diploma with three years.
• Experience with Vermont HCBS Eligibility and Enrollment processes.
• Ability to travel within Windham County and maintain compliance.

Responsibilities

• Coordinate assessment and planning for the individual's services.
• Facilitate ISA planning meetings and invite participants.
• Complete annual needs assessment with the individual and team.
• Identify community resources and supports for plans.
• Monitor ISA/BIP and service plan implementation.
• Ensure documentation is distributed to the support team.
• Advocate for continuity of services and rights.
• Meet reporting and record-keeping requirements.
