Job summary

Medica, a nonprofit health plan, seeks a Case Manager (RN) to educate, coordinate care, and support social determinants of health for Medicare DSNP North Dakota members. You will work with care teams, navigate the health system, and advocate for individuals and families with diverse needs. Remote role; strong clinical documentation and communication skills required.

Qualified candidates have RN licensure, 5+ years in acute/home/community care, and CCM preferred or obtainable within two years.

Qualifications

• The candidate must have an RN license and 5+ years in acute/home/community care.
• Experience with an EHR/clinical documentation platform is required.
• Candidates should have experience working with diverse populations including seniors and those with disabilities.

Responsibilities

• Educate members and coordinate care across the healthcare continuum.
• Collaborate with member's care team and providers to develop care plans.
• Identify and address social determinants of health through community resources.
• Support transitions of care and monitor outcomes against plan of care.
